Contact your insurance provider by calling the customer service phone number on the back of your insurance card. There may be a specific number for mental health/behavioral health services. Asking the following questions is recommended:
- Do my benefits include coverage for behavioral health services?
- Is Dr. Parker an in-network or out-of-network provider for my plan?
- If in-network: What is my co-pay for an outpatient office visit? Do I have co-insurance?
